In case of an incident or accident during a tour with Shaka Isla, this form must be filled out immediately by the guide and the participant(s) involved. It is used to document the facts for insurance and local authorities.
Must be filled out in duplicate (1 for Shaka Isla, 1 for the participant).
⚠️ Legal Obligations:
- This form must be kept for 5 years (legal requirement in Spain).
- In case of a serious accident, call 112 immediately before filling out the form.
